{"id":243833,"date":"2023-01-13T14:41:33","date_gmt":"2023-01-13T19:41:33","guid":{"rendered":"https:\/\/xtramagazine.com\/?p=243833"},"modified":"2023-12-21T10:06:18","modified_gmt":"2023-12-21T15:06:18","slug":"gender-exploratory-therapy","status":"publish","type":"post","link":"https:\/\/xtramagazine.com\/health\/gender-exploratory-therapy-243833","title":{"rendered":"Unpacking \u2018gender exploratory therapy,\u2019 a new form of conversion therapy"},"content":{"rendered":"\n<p class=\"is-style-article-kik\">The practice is sold as supportive care\u2014but in reality, it\u2019s used to delay transition until a person no longer identifies as trans <\/p>\n\n\n\n<p class=\"has-drop-cap\">On June 15, 2022, President Joe Biden released the \u201cExecutive Order on Advancing Equality for Lesbian, Gay, Bisexual, Transgender, Queer and Intersex Individuals.\u201d This order, among other things, established a series of initiatives designed to eliminate conversion therapy for LGBTQ2S+ youth.&nbsp;<\/p>\n\n\n\n<p>One week later, Lisa Marchiano, cofounder of an organization called the Gender Exploratory Therapy Association, <a href=\"https:\/\/www.economist.com.\/united-states\/2022\/06\/23\/the-biden-administrations-confused-embrace-of-trans-rights\" target=\"_blank\" rel=\"noopener\">spoke with<em> The Economist<\/em><\/a> to outline her opposition to the order, saying that she feared a conversion therapy ban could cause a \u201cchilling effect\u201d on the practices promoted by her organization. Marchiano pushes a type of therapy called \u201cgender exploratory therapy,\u201d a practice with an innocuous and even positive-sounding name. Who would be against \u201cexploring\u201d gender in a therapeutic setting?&nbsp;<\/p>\n\n\n\n<p>But this language is purposefully misleading. Dig deeper into the practice and it becomes apparent why its defenders are afraid of conversion therapy bans: gender exploratory therapy is a new form of conversion therapy itself. In sessions, practitioners ask clients\u2014who are almost always youth\u2014to \u201cexplore\u201d the reasons they have gender dysphoria, and encourage them to see their dysphoria as stemming from just about anything other than genuine transness. In fact, desistance from transness is the ultimate goal.<\/p>\n\n\n\n<p>Being trans is <em>never<\/em> an acceptable endpoint to a gender exploratory therapist. Treatments are often held just out of reach until the therapist finds an explanation that is convincing enough to persuade a trans person to delay their transition. Therapists who practise gender exploratory therapy do not intend to ever allow transition for the youth they practise on\u2014<a href=\"https:\/\/twitter.com\/ErinInTheMorn\/status\/1607551924722614273?s=20\" target=\"_blank\" rel=\"noopener\">one therapist listed at the Gender Exploratory Therapy Association\u2019s website<\/a>, JD Robertson, actively maintains that trans people should not be allowed to transition until \u201cfull cognitive development,\u201d which <a href=\"https:\/\/thevelvetchronicle.com\/why-is-nobody-talking-about-cognitive-brain-development-in-the-rush-to-transition-kids\/\" target=\"_blank\" rel=\"noopener\">she defines as 25 years old<\/a>.&nbsp;<\/p>\n\n\n\n<p>This long-term delaying is a common tactic among practitioners who use gender exploratory therapy as a way to avoid letting youth transition before puberty ends. Therapy will often last for years until the trans youth \u201cdesists\u201d from being trans. This has been the case since the start: the original practitioner who coined the phrase \u201cgender exploratory therapy,\u201d Anastassis Spiliadis, a psychotherapist at the South London and Maudsley NHS Foundation Trust, <a href=\"https:\/\/www.researchgate.net\/publication\/334559847_Towards_a_Gender_Exploratory_Model_slowing_things_down_opening_things_up_and_exploring_identity_development\" target=\"_blank\" rel=\"noopener\">described a case<\/a> where he practised this therapy on a trans youth for more than two years without allowing them to transition.<\/p>\n\n\n\n<p>Researchers at the <a href=\"https:\/\/transsafety.network\/posts\/gender-exploratory-nhs-training\/\" target=\"_blank\" rel=\"noopener\">Trans Safety Network<\/a>, a research collective that focuses on threats targeting the trans community, managed to obtain gender exploratory therapy materials from a NHS training in the U.K. held by a consultancy called Explore Consultation. These materials show exactly how the practice of gender exploratory therapy works, and the way it is billed.&nbsp;<\/p>\n\n\n\n<p>In its materials, Explore Consultation claims that a person\u2019s gender dysphoria can be explained by factors other than transness, such as social contagion, mental health issues, autism, shame over sexual orientation and more. These \u201calternative causes\u201d of being trans are then used to delay transition. Sometimes, for instance, a practitioner will say that repressed homosexuality is responsible for \u201cfeeling trans,\u201d and they will explore these feelings over several months while delaying transition. Other times, they\u2019ll state that alternate mental health issues like depression need to be resolved first\u2014a position that found its way into a <a href=\"https:\/\/ohiocapitaljournal.com\/2022\/11\/17\/ohio-bill-limiting-health-care-for-lgbtq-youth-would-create-a-type-of-registry-of-trans-kids-activist-says\/\" target=\"_blank\" rel=\"noopener\">recent Ohio bill<\/a> targeting the healthcare of trans youth. These comorbidities are <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35212746\/\" target=\"_blank\" rel=\"noopener\">often best treated by transitioning<\/a>, and so the delay in transitioning until they are resolved can cause unnecessary suffering. As they wait, trans youth also go through irreversible puberty.&nbsp;<\/p>\n\n\n\n<p>The materials even advocate for barring trans youth from social transition in many cases. This can be dangerous, as we know that providing gender-affirming care <a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2789423\" target=\"_blank\" rel=\"noopener\">has been shown to reduce self-harm and suicidal ideation<\/a> among trans youth by up to 73 percent.<\/p>\n\n\n\n<p>Gender exploratory therapy utilizes gaslighting and delay techniques in order to obtain an outcome that is desirable for those ideologically opposed to transition. In many ways, these techniques are similar to crisis pregnancy centres that also seek to <a href=\"https:\/\/www.focusonthefamily.com\/family-qa\/exploring-options-for-an-unwed-pregnant-teen\/\" target=\"_blank\" rel=\"noopener\">\u201cexplore options\u201d<\/a> around pregnancy and \u201cprovide counselling\u201d that has been <a href=\"https:\/\/maryland.prochoiceamericaaffiliates.org\/wp-content\/uploads\/sites\/11\/2018\/04\/crisispregnancycenterreport.pdf\" target=\"_blank\" rel=\"noopener\">described by pro-choice organization NARAL<\/a> as \u201cbiased, manipulative, and misleading\u201d and utilizing \u201cdelay tactics to deter and prevent\u201d abortions. It is no surprise then that religious groups like the Catholic Psychotherapy Association also publish materials that directly link to the <a href=\"https:\/\/www.catholicpsychotherapy.org\/resources\/2022%20Conference%20Handouts\/Integrating%20Body%20and%20Soul%20-%20Closing%20Plenary\/Gender%20Dysphoria%20Resources.pdf\" target=\"_blank\" rel=\"noopener\">Gender Exploratory Therapy Association<\/a>.<\/p>\n\n\n\n<p>In recent years, gender exploratory therapy has hit the mainstream. In late 2021, prominent proponents of the practice, Dr. Erica Anderson and Laura Edwards-Leepers, <a href=\"https:\/\/www.washingtonpost.com\/outlook\/2021\/11\/24\/trans-kids-therapy-psychologist\/\" target=\"_blank\" rel=\"noopener\">published an article<\/a> in <em>The Washington Post<\/em> explicitly advocating for gender exploratory therapy. Both of these proponents also appeared in an article in <em>The Atlantic<\/em> titled <a href=\"https:\/\/www.theatlantic.com\/magazine\/archive\/2018\/07\/when-a-child-says-shes-trans\/561749\/\" target=\"_blank\" rel=\"noopener\">\u201cWhen Children Say They\u2019re Trans,\u201d<\/a> where Edwards-Leeper encouraged \u201cexploration with a little more nuance.\u201d The article was heavily criticized by trans rights activists and organizations such as the GLAAD Accountability Project for focusing on rare cases of trans regret, misinterpreting studies and claiming that trauma and social contagion can cause trans identification.<\/p>\n\n\n\n<p>The practice\u2019s proponents have even begun to inform health policy in the U.S. Florida recently decided to ban Medicaid coverage for gender-affirming care, for example, as well gender-affirming care for trans youth. In documents related to both bans, gender exploratory therapy was listed as an alternate way of dealing with gender dysphoria.<\/p>\n\n\n\n<p>Multiple medical organizations like the <a href=\"https:\/\/publications.aap.org\/pediatrics\/article\/142\/4\/e20182162\/37381\/Ensuring-Comprehensive-Care-and-Support-for\" target=\"_blank\" rel=\"noopener\">American Academy of Pediatrics<\/a> and the <a href=\"https:\/\/searchlf.ama-assn.org\/letter\/documentDownload?uri=%2Funstructured%2Fbinary%2Fletter%2FLETTERS%2F2021-4-26-Bill-McBride-opposing-anti-trans-bills-Final.pdf\" target=\"_blank\" rel=\"noopener\">American Medical Association<\/a> have spoken out against \u201cwait and see\u201d approaches that seek to delay or deny transition.&nbsp;<\/p>\n\n\n\n<p>Still, gender exploratory therapy tries to paint itself as a value neutral third option between allowing youth to transition and denying them care outright. Practitioners claim that they do not seek any end goal for their patients. The Gender Exploratory Therapy Association\u2019s <a href=\"https:\/\/genderexploratory.com\/\" target=\"_blank\" rel=\"noopener\">welcome message<\/a> states that it seeks to \u201cprovide thoughtful care without pushing ideological or political agendas.\u201d&nbsp;<\/p>\n\n\n\n<p>Just underneath the welcome message, though, is a link to <a href=\"https:\/\/genderexploratory.com\/2022\/10\/28\/geta-submits-a-comment-on-title-ix-rule-making\/\" target=\"_blank\" rel=\"noopener\">a brief the organization filed<\/a> opposing President Biden\u2019s Title IX changes that protect trans students from discrimination. This brief is a steadfastly political document that, among other things, states that trans youth in restrooms would harm the mental health of their peers and therefore they should be banned from restrooms matching their gender identity. It is clear that the proponents of this \u201ctherapy\u201d have a political agenda.<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>\u201cOne recipient was told by their therapist that their pronouns would not be respected because their transness was a response to trauma.\u201d <\/p><\/blockquote><\/figure>\n\n\n\n<p>In a recent TikTok where <a href=\"https:\/\/www.tiktok.com\/@erininthemorn\/video\/7179719572893781294?is_copy_url=1&amp;is_from_webapp=v1\" target=\"_blank\" rel=\"noopener\">I described the practice<\/a>, dozens of trans youth and adults recounted their own experience with gender exploratory therapy. One person described an experience where they were told by their therapist that their pronouns would not be respected because their transness was a response to trauma. Another user was told that their trans male identity was actually a result of the person feeling unsafe as a girl.&nbsp;<\/p>\n\n\n\n<p>I posted on <a href=\"https:\/\/twitter.com\/ErinInTheMorn\/status\/1605222777061244929?s=20\" target=\"_blank\" rel=\"noopener\">Twitter <\/a>asking for people\u2019s experiences with gender exploratory therapy, and nearly a hundred people responded. One recounted being told <a href=\"https:\/\/twitter.com\/SmartAssJen\/status\/1605302422502445057?s=20\" target=\"_blank\" rel=\"noopener\">she was just a repressed homosexual<\/a> while another described a nearly <a href=\"https:\/\/twitter.com\/CamomileOgden\/status\/1605224415310094338?s=20\" target=\"_blank\" rel=\"noopener\">four-year fishing expedition<\/a> into other reasons why she might be trans. Many of these people have since transitioned and have dealt with the trauma of psychological gaslighting.<\/p>\n\n\n\n<p>I spoke with one such trans person who went through exploratory therapy. Alex* (who requested a pseudonym in order to protect them from potential online harassment) went through years of exploratory therapy with a therapist who implied that their gender identity dysphoria was a product of their struggles with mental health, and not the other way around., \u201cEssentially, if I still had anxiety issues or was struggling in school, or hadn\u2019t fully worked through childhood trauma (in their eyes), that meant I couldn\u2019t know if I was actually trans,\u201d they explain.<\/p>\n\n\n\n<p>Alex says they felt gaslit by the process and realized that any problems they brought to their therapist would be used to discredit their gender identity. \u201cIt got to a point where I was engaging in self-harm, but couldn\u2019t tell my therapist because I knew it would \u2018set me back\u2019 in the process that I can now see was actually a never-ending cycle.\u201d<\/p>\n\n\n\n<p>Meanwhile, states continue to use gender exploratory therapy to justify banning gender-affirming care for trans youth and even some trans adults. Affirming parents may not recognize this and send their children to gender exploratory therapists with the expectation that the therapist will help their child explore their gender identity, when in reality the therapist seeks to gaslight their child and steer them away from ever transitioning. The damage this causes to the mental health of those exposed to it is immense and readily apparent by those who have suffered it. \u201cBecause of the therapy I received, I never got to socially transition in high school, and it made transition in college much much more difficult,\u201d Alex said.<\/p>\n\n\n\n<p class=\"is-style-end\">Alex explained that if they had received more affirming therapy, they never would have felt the need to hide their self-harm.
